Course/Program PlanDENMARK—AGRICULTURE: This course covers an introduction to agricultural, food, and environmental systems in other countries of the world. The field study seminar consists of two major components: a series of pre-departure seminars and a two-week period within a host country. The field study will introduce students to the history, geography, culture and socio-economic situation of the country they will visit. Students will also explore the agricultural, food, and environmental system of another country. Program and Tuition Fees
Program Fee: Applications must be accompanied by a deposit check of $100.00 payable to CCID. The Program Fee includes round-trip airfare to/from CCID designated departure airport to international gateway to destination city and return; Emergency Evacuation and Health Insurance; in-country group transfers; lodging and most meals. Enrollment is limited. A $100.00 nonrefundable deposit (applied toward the Program Fee) must accompany your application to reserve you a place in the program. Qualified applicants must pay the Program Fee in full by the deadline to secure their participation in the program. After this date, wait-listed students will be invited to participate on a space available basis and provided they can make full payment. Tuition Fee: In addition to the Program Fee, students must enroll in the course designated by their home institution or participating college. Following your school’s standard procedures, students should register and pay tuition for the course. Contact instuctor at your college to inquire about the course enrollment details. The Program Fee and Tuition Fee do not include the following: books, passport and visa fees, immunizations, airport departure taxes, personal expenses and anything not specified as included in the program. Scholarships: CCID does not award scholarships nor does it guarauntee participating colleges awarding of scholarships. WAIVER, RELEASE AND INDEMNIFICATION AGREEMENTI understand that CCID and [other program participant colleges] may have to share educational data about me as part of my participation in this Program. I authorize CCID [or other program participant colleges] to share such educational data about me, including with my emergency contact identified above, that are necessary to process my application, evaluate my performance, complete the Program, and/or to assure my health or safety during the Program. I understand that if I do not consent to the sharing of data about me, I cannot participate in this program. I certify that all information is true and to the best of my knowledge. By signing this form, I give CCID [or other program participant colleges] the right to access my records, transcriptions and any disciplinary records. I understand that if I am accepted for a study abroad program, that I will be a representative of CCID [or other program participant colleges] and be subject to its rules and regulations we well as, the rules and regulations of any host institutions abroad. I understand and agree to the above waiver, release and indemnification agreement. MODEL RELEASEI hereby consent to and authorize the use and reproduction by CCID or anyone authorized by CCID, of any and all photographs, video and radio interviews, and surveys, which CCID has taken of my self, for any purpose whatsoever, without compensation to me. CCID may use the photographs/quotes from surveys, for promotional and information purposes through publications, broadcasts, and website for a period of two (2) years from the date of my signature. I understand and agree to the above model release. MEDICAL INFORMATIONI understand that by applying for the Program and by paying the full Program Fee, additional overseas Emergency Evacuation and Medical Insurance is to be obtained for me through CCID Office. I understand that CCID does not have medical personnel available at the location of the Program, during transportation, at the Host Institution/Affiliate or anywhere else where I may visit while a Participant in this Program. I am responsible for all of my medical needs and medications while participating in the Program, and am not relying upon anyone else therefore. I agree that at its discretion, CCID [or other program participant colleges] may share my Student Medical Report with the Host Institution/Affiliate, and/or Program Faculty. I hereby consent to such disclosure of this Medical Report in connection with my participation in the Program, pursuant to the Family Educational Rights and Privacy Act of 1974, as amended. CANCELLATION AND REFUND POLICYParticipants must inform CCID in writing if they choose to cancel their participation in the program and accept that the effective date is the postmark date on the envelope. All correspondence should be addressed to the lead faculty member. Every effort will be made to accommodate substitutions for students who withdraw. Applicants must understand that the CCID is required, however, to make various deposits to reserve accommodations, air tickets, in-country travel, etc. Applicants must accept that withdrawal from the program or failure to pay any outstanding balance on their accounts by deadlines provided in supplemental materials, therefore, will be subject to the following refund and penalty schedule and conditions: Withdrawal Refund and penalty amounts 60 or more days prior to departure Full refund less a $100 deposit and nonrefundable air an land deposits. 59 to 45 days prior to departure Full refund less a $350 fee and any nonrefundable air and land deposits. 44 to 31 days prior to departure Full refund less a $700 fee and any nonrefundable air and land deposits 30 or fewer days prior to departure Full refund less a $1000 fee and any nonrefundable air and land deposits CCID will periodically assess enrollment and other factors. Students accept that the CCID has the right, at any time and without liability or cost therefore, to cancel the program. If the program is cancelled prior to departure, participants enrolled in the program at the time such a decision is made will receive a full refund. Applicants who withdrew voluntarily prior to any decision to cancel the program are subject to the cancellation and refund policy above.

STUDENT’S/PARENT’S CHECKLIST[ ] Immediately mail to CCID a completed and signed Application (p.2) including the Waiver, Release & Indemnification Agreement and Medical Release Statement (p.3)
[ ] Apply for a Passport (if you do not have one) and visa (if applicable) immediately upon the confirmation of your participation in the Program. [ ] Register and pay tuition for the study abroad course as you would for a regular course at your College. Please contact the contact instructor at your college and/or the Lead Program Instructor to inquire about the specifics of this registration. [ ] Pay the Program Fee in full by March 15, 2005. Make your check payable to CCID, and mail at the address provided. [ ] Consult your personal physician regarding immunizations as well as other health and safety concerns at least 8 weeks prior to departure.
